Psychiatrist, psychologist or counsellor: who should you see?
The titles are confusing everywhere, and India adds its own registers and rules. Here is who does what, who can prescribe, and how to pick a starting point without worrying about getting it wrong.
Psychiatrists: medical doctors
A psychiatrist completed MBBS and then an MD or DNB in Psychiatry. They can diagnose, prescribe and manage medication, and they are the right first stop when symptoms are severe: when sleep, appetite or daily functioning have broken down, or when you already suspect medication may be part of the answer.
Clinical and rehabilitation psychologists: trained in therapy and assessment
A Clinical Psychologist holds an M.Phil or Psy.D in Clinical Psychology and an RCI registration. They are trained in psychological assessment and structured psychotherapy for diagnosed conditions. They do not prescribe medication.
Rehabilitation Psychologists are registered with the RCI in their own category and work with disability, neurodevelopmental conditions and rehabilitation after illness or injury.
Counsellors: support for life's weight
Counsellors typically hold a Master's in Psychology. They work with stress, relationships, grief, career and adjustment: the heavy parts of ordinary life. They do not diagnose conditions or prescribe medication, and the good ones are clear about that line.
They work together, not in competition
A counsellor who suspects clinical depression refers to a psychiatrist or clinical psychologist. A psychiatrist who prescribes often recommends therapy alongside, and research supports the combination for many conditions. Referral is what good practice looks like, not a sign you chose wrong.
So where do you start?
Start wherever feels accessible: in cost, in distance, in comfort. If your symptoms feel severe or physical, start with a psychiatrist. Otherwise, any verified professional is a reasonable first door, because a good one will tell you if you need a different kind of help.
Common questions
Do I need a referral to see any of them?
No. In India you can book a psychiatrist, psychologist or counsellor directly, no referral letter required.
Who can prescribe medication?
Only psychiatrists, because they are medical doctors. Psychologists and counsellors cannot, whatever their experience.
Can I start with a counsellor if I might have depression?
Yes. A trustworthy counsellor screens for severity and refers you to a psychiatrist or clinical psychologist if what you are carrying needs clinical care.
