Your first therapy session: what actually happens

For most people the hardest session is the one before the first: the booking. Much of that fear is simply not knowing what happens in the room. So here is what happens.

Before the session

There is nothing to prepare. You do not need the right words, a diagnosis, or a tidy story. "I have not been feeling like myself and I am not sure why" is a complete and common opening.

What the first conversation covers

Expect gentle history-taking: what brings you now, how long things have felt this way, sleep, work, relationships, what support looks like around you. You set the pace, and you can decline any question. The first session is also you assessing them: how it feels to talk to this person is real information.

Confidentiality, honestly

What you share is confidential. The honest version includes the limits: if there is serious risk of harm to you or to someone else, or a legal requirement, a professional may need to involve others to keep people safe. A good therapist explains these limits up front without being asked.

If it does not click

The relationship with your therapist is one of the strongest predictors of whether therapy helps. If after a session or two it does not feel workable, trying someone else is normal practice, not failure, and no good therapist resents it.

Common questions

Will I have to talk about my childhood?

Only if and when you choose to. You control what you share; a first session works entirely at your pace.

What if I cry, or go blank?

Both happen in first sessions every week of a therapist's working life. There is no performance standard to meet; silence and tears are workable material, not problems.

Is it okay to do the first session online?

Yes. Many people find it easier to start from their own room. You can move to in-person later if you prefer.
